US benefits: carriers and recordkeepers, EDI 834 and 401(k)/457 files, ACA 1095-C and 1094-C, garnishments, and the Benefits Dashboard

United States companies only: send each insurer its ANSI 834 enrollment file and each 401(k) or 457 recordkeeper its contribution file after payroll, by download, HTTPS or SFTP to a pinned host key; measure ACA full-time status and produce Forms 1095-C and 1094-C, which employees open in self service; take garnishment orders in priority order and remit them per payee. The Benefits Dashboard, for every company, shows enrolment and what needs action.

Carriers and recordkeepers

  1. Carriers & Recordkeepers › New: Code, Name (for example <Aflac> or <Vanguard 401(k)>), Kind (insurance or retirement) and File it takes — ANSI X12 834 enrollment (005010X220A1), Contribution and loan file (CSV, after payroll) or Eligibility file (CSV).
  2. For an 834: the ISA sender and receiver, Test or production (T until the carrier has accepted a test file), Plan sponsor, Sponsor EIN, Insurer and Master policy / group number. For a CSV: Recordkeeper plan number, Deduction codes by source, Columns, Delimiter and Dates; tick Build the file when a payroll run is posted.
  3. Plans covered lists the plan codes the carrier is sent.
  4. Delivered by: Download and upload to the carrier portal, HTTPS to the carrier’s endpoint (https only — files carry Social Security numbers) or SFTP. For SFTP: Set credential, Fetch host key, compare it with what the carrier publishes, then Pin host key. Nothing is sent until a key is pinned.

Carrier Files

  • Build a file: Carrier and File — Full enrollment (834), Enrollment changes (834), Contributions and loans (from an approved or posted Payroll run) or Eligibility — and Send it as soon as it is built.
  • A change file compares with the last enrollment file sent: “Send a full file first” if none was, and “No change file is needed” when nothing changed.
  • Each row: Download, Send (Resend once sent), Regenerate a corrected version, Log (every delivery attempt and download) and Void (only before it is sent). Files are kept sealed.
  • A recordkeeper with Build the file when a payroll run is posted gets its contribution file at posting, sent at once unless it is delivered by download.

ACA: settings, full-time status and forms

  1. ACA Settings (one per company): Full-time by Look-back measurement or Monthly hours (130 a month); the measurement, administrative and stability periods; the affordability safe harbor (Rate of pay (2H), Form W-2 wages (2F), Federal poverty line (2G)); and the employer on the 1094-C. Plan ACA flags marks a plan Minimum essential coverage, Minimum value and Self-insured.
  2. ACA Full-Time Status: choose the Year to see each employee FT, PT or LNA month by month, with Why. Keep the measurements records them.
  3. ACA Forms: Tax year, Generate drafts (lines 14, 15 and 16 for every month and the 1094-C), check with Why and Print, then Finalize — employees then see their form. Correct a final form to issue a corrected one. AIR XML downloads the file shaped to the IRS AIR schema for your transmitter.

Employee Self Service › Me › My Workspace › My Tax Forms lists the employee’s final Form 1095-C by tax year; Open shows the PDF.

Garnishments

  • Garnishment Orders: Employee, Garnishment code (its max % of disposable earnings is the CCPA limit), Kind of order, Case number, Amount a period or percent, Stop after (arrears), Priority, Received, Pay to and Sent by (Check, EFT, or EFT to a state disbursement unit, which needs the SDU FIPS code).
  • Payroll takes orders in priority order — empty, by kind: support first, then levies, bankruptcy, student loans, creditors — and among equals the order received first.
  • Garnishment Remittances: after an approved or posted run choose Prepare — one remittance per payee, its file carrying the DED addenda for child support to a state disbursement unit. Remitted records the Payment reference (check number or EFT trace); Void a prepared one that is wrong.

Benefits Dashboard

Employees eligible, Enrolled in a plan, Enrollment rate, No election or waiver, Open enrollment progress, Life events open and Dependents awaiting proof (Verified or Reject on the spot), with Alerts and Enrollment by plan. For a United States company it adds ACA full time, Full time offered coverage, Carrier files failed, ACA compliance and garnishments withheld but not yet prepared. Every figure links to its list.

How to reuse: carrier names, codes and countries in angle brackets are examples; replace them with your own.
