Recording when a clinical event happened
Assessments, immunizations, problems, allergies, results, records requests and outside medications can be given their real date and time instead of the time they were entered.
- Behavioral Assessments: Administered. For example, a PHQ-9 series keeps each assessment's own date.
- Immunizations: Given. Use this for a dose given elsewhere or last week.
- Problem List and Allergies: Recorded.
- Enter result on a clinical order: Observed.
- Records requests: Received. This is the date the request arrived, and the response clock starts from it.
- Outside Medications: Received.
If you leave the field empty, the current date and time is used. A date in the future is refused, because these records describe something that has already happened. Prescribing a medication through the safety check also accepts a start date.
