Reconcile a patient's medications

Line up what the patient takes at home, what is ordered here and what another system sent; decide each; sign; the medication list changes to match.

1. Open Clinical > Medication Reconciliation and press Start, choosing the patient and the transition.
2. Open Side by side to see each medicine from each list and the discrepancy found.
3. Press Decide and choose continue, modify (with the new dose or frequency), hold, discontinue or new for each.
4. Add a medicine started now with Add a medicine.
5. Sign. Every decision is applied; anything still undecided is flagged as unresolved.
6. Print for the patient gives the reconciled list to take home.
